A 241-Am/Be probe was lost down a well, and the probe's containment was damaged in the recovery process.

Amounts of radiative material released was not disclosed, although the probe originally was very radioactive.

Although the location (NM?) of this nuclear “accident” was not disclosed, this should raise drinking water concerns when neutron and other well-logging is used and abused.

Epithermal neutron well-logging is unwise in water-bearing Dakota Sandstone, Morrison Formation, Entrada Sandstone, Chinle Group, Cutler Formation, and Madera Limestone containing fertile 238-U or depleted uranium.
